Calendar Event Form

  • Fill out this form to request a date/time change of a current event, to add a new  event to the church calendar or to cancel an event date(s).
  • Please give ample time for your request to be processed.

Use Request Form (URF)

  • Use this form to submit a request to use the church building and/or items from the church for a personal/private event, whether at the church building or a different location.
  • Please give ample time for your request to be processed.


Purchase Request Form


  • Click the above link if you would like to submit a PURCHASE REQUEST. 
  • So there is no delay in your request be sure to fill out as much as possible.
  • Then save to your computer, attach to an email and send to caa@fhcCheyenne.org
  • Your request will be processed as quickly as possible and can take up to a week.
  • The original that is sent will be printed off and placed in your box at the church when process is complete.


Announcement Request Form

  • Click on the above link to submit an announcement request.
  • Announcements must be approved by Pastor before being published by the Church Staff.

Team Member of the Month Nomination

  • Click the above link if you would like to submit a Team Member of the Month Nomination.  
  • Nominations are due by the first Tuesday of each month and can be turned in by any Staff Member.
